My clinical research has focused on the relationship between intra-atrial conduction abnormalities and susceptibility to atrial fibrillation (AF). In patients undergoing electrophysiological studies we have identified a critical area of slow and non-uniform anisotropic conduction located at the base of the right atrium, which is strongly associated with induction of AF. We have also made the observation that continuous pacing of the left atrium via the coronary sinus can prevent AF in patients susceptible to AF induction by atrial extrasystoles. My basic research efforts aim to explore the molecular mechanisms of arrhythmogenesis. I have postulated that enhanced anion-exchange activity in the myocardium may mediate triggered arrhythmias, and I have worked toward the characterization of expression and function of anion-exchanger (AE) isoforms in cardiac tissue. I have expressed the rat cardiac AE3-isoform cDNA product in Xenopus oocytes, and have studied its regulation by extracellular pH changes and by cytoplasmic AE peptides. In addition, I have maintained a mouse atrial tumor cell line (AT-1/HL-1 cells), and have performed a full characterization of expression and function of AE isoforms in AT-1 cells, including the regulation of AE mechanisms by paracrine factors.
